Job Summary :  To perform medication history assessments for new admissions to the hospital and provide complete information regarding each medication identified (i.e. drug, dose, route, frequency, indication for PRN, and date/time of last dose taken). Medications to be reviewed include prescription and over the counter medications (including herbals, vitamins and dietary supplements).
