REGISTERED NURSE

Gifted Healthcare is seeking a PRN LTAC RN to deliver high-quality, complex nursing care in a long-term acute care setting. In this flexible, per diem role, you will assess, plan, implement, and evaluate care for medically fragile, high-acuity patients requiring extended recovery. Responsibilities include monitoring vitals and telemetry, managing ventilators and trachs, administering medications and IV therapies, coordinating multidisciplinary care, educating patients and families, and documenting in the EMR. Responsibilities

• Provide comprehensive nursing care to high-acuity LTAC patients on a PRN basis
• Assess, plan, implement, and evaluate individualized patient care plans
• Monitor vital signs, telemetry, and patient response to treatments and interventions
• Administer medications, IV therapies, and manage central lines per protocol
• Care for ventilator and tracheostomy patients, including monitoring and troubleshooting
• Perform complex wound care, dressing changes, and prevention of pressure injuries
• Collaborate with physicians, respiratory therapy, and interdisciplinary team members
• Educate patients and families on conditions, treatments, and discharge planning
• Document assessments, interventions, and outcomes accurately in the EMRAdhere to infection control, patient safety, and regulatory standards
