Family Practice/Primary Care Nurse Practitioner

Job Description

We are seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver high-quality Primary Care within a Value-Based Care (VBC) model. This role is ideal for an APP experienced in managing chronic conditions, improving outcomes, and supporting population health initiatives through coordinated, proactive care.

Required Experience (Value-Based Care)

Nurse Practitioner candidates must have prior Value-Based Care experience, including experience with quality measures, risk-based/panel management, care coordination, outcomes-based care delivery, and documentation practices that support accurate clinical capture and continuity of care.

Preferred Clinical Experience (Renal/Nephrology)

Renal and/or nephrology experience is highly preferred, including familiarity with CKD management, hypertension, diabetes-related renal disease, electrolyte issues, and coordination of care for patients with renal impairment (including dialysis coordination when applicable).

QualificationsEducation

  • Nurse Practitioner: Master s degree in Nursing (MSN) from an accredited program and completion of an accredited NP program.
  • Physician Assistant: Graduate of an accredited PA program.

Licensure/Certification

  • Current, unrestricted Florida licensure as an ARNP or PA (as applicable).
  • Current national board certification in the applicable discipline.

Clinical Background

  • Strong foundation in Primary Care (Internal Medicine and/or Family Medicine preferred).
  • Comfort managing medically complex, chronically ill patient populations.

Key Responsibilities

  • Provide comprehensive Primary Care services, including assessment, diagnosis, treatment, and longitudinal management of acute and chronic conditions.
  • Practice in a Value-Based Care framework, supporting quality initiatives, prevention/screenings, patient engagement, and measurable health outcomes.
  • Coordinate care with specialists, ancillary services, and internal teams to ensure seamless care transitions and follow-through on care plans.
  • Support management of patients with renal disease as needed, including timely referrals, monitoring, and collaboration with nephrology.
  • Educate patients and families on lifestyle, medications, prevention, and disease self-management.
  • Maintain timely, accurate, and detailed documentation in the medical record.
