We are seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver high-quality Primary Care within a Value-Based Care (VBC) model. This role is ideal for an APP experienced in managing chronic conditions, improving outcomes, and supporting population health initiatives through coordinated, proactive care. Required Experience (Value-Based Care) Nurse Practitioner candidates must have prior Value-Based Care experience , including experience with quality measures, risk-based/panel management, care coordination, outcomes-based care delivery, and documentation practices that support accurate clinical capture and continuity of care.
