How You’ll Help Transform Healthcare: This position will be servicing established patients in their homes through a Mobile Health unit. The patients are identified as those who have fallen in the gaps of care of medical management. The gaps include managing chronic diseases, Medicare wellness and preventative care, vaccine hesitancy, medication compliance, safety, etc.  Visits will be scheduled in the patient's home, where the nurse will do a basic nursing assessment to include vital signs, review of medications, patient history, etc. The nurse will be provided with a Carilion van for travel that will be equipped with Point of Care testing units and supplies, as well as lab supplies to perform phlebotomy work. The nurse will also have access to TytoCare devices to assist a virtualist provider in performing assessments and evaluation of the patient.
