Job Summary:  Coordinates all clinical nursing aspects of resident’s admissions and discharges. Acts as liaison between physician, rehab and nursing department to assure resident safety through these processes. Is responsible for completion of all nursing admissions/discharge orders and documentation related to admission and discharge processes as well as for validation of completion. Participates in quality assurance and quality improvement as appropriate for member of Courtland Terrace leadership team. Other duties as assigned. Performs all duties in compliance with Courtland Terrace policy and procedures as well as in compliance with long-term care regulatory agencies. Is supportive of corporate and facility mission statement, goals and values.
