Summary JOB SUMMARY/PURPOSE Provides clinical pharmacy services to ambulatory patients. Provides comprehensive medication management in multiple areas of practice, including but not limited to anticoagulation, perioperative management of anticoagulants and antiplatelets, anemia management with erythropoiesis stimulating agents, diabetes,  hypertension, hyperlipidemia, heart failure, COPD, asthma and opioid use disorder . Ensures departmental efficiencies and compliance with regulatory bodies.
