Many of our patients come to us directly from the ICU or the emergency department, at times creating the atmosphere of a medical step-down unit. We also have patients admitted to us from ICUs in outlying community hospitals throughout Vermont and northern New York. We are primarily coaching and educating our patients as we care for their medical needs.  We strive to help our patient’s transition back home by helping to guide, coach and direct them to become independent in their activities of daily living.
