Direct Care and Treatment (DCT) is a highly specialized behavioral health care system that serves people with mental illness, substance use disorders, intellectual disabilities, and other serious and often co-occurring conditions. DCT operates psychiatric hospitals and other inpatient mental health treatment facilities; inpatient substance abuse treatment facilities; special-care dental clinics; group homes and vocational sites; and the nation’s largest treatment program for civilly committed sex offenders. The system cares for more than 12,000 patients and clients each year. Because these individuals have conditions that are complex and behaviors that can be challenging, other health care systems cannot or will not serve them. The RN Care Manager (Care Coordinator/ACO) is responsible for working with Coulee Medical Center's medical teams to provide quality, comprehensive care to patients with complex health situations. This position’s duties include individual screening, chronic condition(s) education, medical assessments, charting, care planning, stabilization, observation and maintenance, ongoing coordination of follow-up care, and support for an individual’s self-management. The RN Care Manager empowers patients to self-manage their condition, reduce the occurrence of preventable complications, and overall help patients stay out of the hospital. This position will also serve as the ‘population health nurse’ for CMC's ACO program. Remote/hybrid schedule available, with expectations of being onsite at Coulee Medical Center twice weekly.
