About the Job General Summary of Position Assesses, plans, implements, and evaluates the quality of care to patients during episodic and chronic visits. Assesses, plans, implements, and evaluates the quality of patient care.   Primary Duties and Responsibilities   Attends all scheduled meetings including but not limited to regular staff meetings and training classes for safety infection control OSHA CPR and others as required. Collaborates as part of the primary medical care team to formulate and document ambulatory care plans for patients and families.
