Overview Assists the clinical pharmacists in providing an accurate and timely medication distribution system.  Qualifications Education: High School Diploma or GED required. License: Must be registered with the Alabama State Board of Pharmacy as a Pharmacy Technician within 30 days of hire date, and registration must be in good standing “active”.  Employees must obtain national certification with PTCB (Pharmacy Technician Certification Board) or ExCPT (Exam for the Certification of Pharmacy Technicians) within one year of hire date.  In the case of initial certification by examination, a paper result from the PTCB or ExCPT examination of “pass” will be considered active status for 30 days.  Additional Skills/Abilities: Must have excellent communication skills with the ability to work in a fast-paced environment requiring prioritizing and changing tasks frequently and quickly. Overview Promotes optimal patient outcomes by assisting with medication reconciliation.  Interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to the hospital.  Compiled history information is entered into the patient’s electronic health record for the purpose of facilitating medication reconciliation by the admitting provider.  May perform a variety of functions which impact the medication reconciliation process and patient care.  Seeks assistance/clarification from a pharmacist with any questions or issues that may arise.
