DESCRIPTION OF POSITION: Under the direct supervision of the Outpatient Rehab Manager, the Physical Therapist evaluates and treats all patients referred for physical therapy in the Outpatient setting; prepares reports and communicates patient progress to MDs and other clinical staff; contributes to the development of the department. SUPERVISION: Reports directly to the Director of Physical Medicine RESPONSIBILITIES AND DUTIES: Provide direct patient care. Designs and implements a plan of care for the patient based on assessment. SUPERVISION: Reports directly to the Director of Physical Medicine RESPONSIBILITIES AND DUTIES: Completes documentation outcome measures for evaluations, daily, weekly progress notes and discharge summaries as required by IRF regulations. Assesses the functional needs of the patient and sets appropriate goals for that patient. Provides direct patient care; evaluates outcomes; and consults with other specialists, as needed. Provides patient/family with options for adaptive equipment. Oversees the care of all assigned patients and all care givers rendering such care, including support staff, volunteers and students.
