Assists patients as necessary (e.g. ADL’s, feeding) by providing high quality, compassionate patient care Maintains on-going communication with RN and LPN regarding patient status, care given and patient response. Documents care, vital signs and all other pertinent information legibly and accurately on patient’s flow sheet/graphic sheet. Communicates appropriate information to patients and families. Ensures that care information is transmitted in a timely, clear, and accurate form to all patients’ current healthcare providers who need that information to provide care.
