The night shift Critical Care Advanced Practice Provider ( APP ) functions within the adult ICU areas and is an integral member of a multidisciplinary health care team. The APP functions primarily managing critically ill adults and serves as a resource throughout UKHC for intensive care interventions. The APP is generally on a service aligned team and manages the day to day patient care from ICU to step down to discharge. In some instances the APP may have responsibilities for this same patient population as they return for follow up in the outpatient setting. The APP also may play a role in the admissions process including ICU triage. The Ambulatory Nephrology APP functions primarily in the ambulatory setting, caring for nephrology patients’ needs, including kidney transplant.  The APP is an integral member of a multidisciplinary health care team. The APP functions in the ambulatory setting, providing comprehensive outpatient care across the continuum to manage acute and chronic needs of this population. This APP is generally service-aligned and works closely with the Nephrology medical team.  The APP has a strong foundation in evidenced based practice and education and has a responsibility for ensuring an environment of safe quality care. The Advanced Practice Provider ( APP ) in the Memory Disorders clinic will provide comprehensive evaluation and management of patients with cognitive and memory concerns. Working collaboratively with physicians, nursing staff, and other interdisciplinary team members, the APP will conduct detailed patient assessments, perform neurological and cognitive screenings, formulate differential diagnoses, and develop evidence-based care plans. Responsibilities include managing ongoing treatment, coordinating referrals, educating patients and families about disease progression and care options, and contributing to clinical research or quality improvement initiatives related to memory disorders. The APP will play an integral role in optimizing patient outcomes and supporting the mission of providing compassionate, specialized care for individuals with memory and cognitive impairments.
