Job Summary:  The Heart Failure Nurse Navigator coordinates evidence-based care for patients with heart failure across inpatient, transitional, ambulatory, community, and virtual-care settings. Serving as a patient advocate and liaison among patients, families, providers, and interdisciplinary teams, the navigator identifies eligible patients, closes gaps in care, supports timely follow-up, promotes effective self-management, and advances quality outcomes, including reduced preventable readmissions and improved patient experience. This position coordinates up till discharge and then passes off to the Heart Failure Transitional Care Nurse for Post-Discharge Care Coordination.

The Nurse Navigator will work with the patient and members of the clinical care team to help navigate their way to timely and quality care and to assist in overcoming any barriers to care. This will include, but is not limited to, connecting them to resources and information to make informed decisions, providing education and emotional support, and coordinating team communications. The NN will assist the patient from initial diagnosis through diagnostic testing, treatment, and follow-up or end-of-life care. This position is in an Oncology Ambulatory Clinic.

Works with the Nurse Navigator in managing OPAT in ID Clinic, proactively participates in patient care rounds/clinic visits, reviewing patient profiles, providing antimicrobial pharmacotherapeutic consults, making patient specific antimicrobial drug therapy recommendations. Identifying and engaging appropriate multidisciplinary clinical experts to enhance judicious use of antimicrobials; identifying, tracking, and measuring both process and outcome measures related to antimicrobial stewardship; and providing comprehensive antimicrobial stewardship education. It will be a position that rotates with other clinical ID pharmacists between ambulatory OPAT /ID clinic and inpatient ID consult teams and stewardship activities.
