The 7N inpatient behavioral health unit has 30 beds with an average length of stay under 8 days. The Senior Psych Social Worker will assist patients and families to define social, emotional and environmental needs and to enable them to recognize/utilize available resources to resolve needs by providing a full spectrum of social work service. In addition, they will be responsible for the coordination of care, family therapy sessions, group therapy sessions, individual therapy sessions, safety planning, discharge planning, admission biopsychosocials, timely transmission of transition records, compliance with federal and state mandates for aftercare planning, treatment team, and support with involuntary commitments including court hearing. RN Case Manager – Weekender - Day Shift Saturday & Sunday plus one rotating Friday Shift  Newark, DE ChristianaCare - Wilmington Hospital is currently recruiting an RN Case Manager - Weekender with experience in Transitional Care or Discharge Planning in an Acute Care Hospital Setting As part of our Weekend Incentive Program (WIP), you will be required to work 3 out of 4 weekends in a four-week schedule. This includes a 12-hour day shift on Saturdays and Sundays, along with one rotating Friday shift.  Additionally, there will be an annual rotating holiday every other year.
