The GeriPACT team is a collaborative partnership with an interdisciplinary team of suitably prepared health care professionals, consisting of the GeriPACT provider, a registered nurse, a social worker, a Clinical Pharmacist, a clinical associate (LPN, LVN or health tech), dietitian, mental health providers, rehab professionals, chaplains, and clerical staff. This group is referred to as the "GeriPACT Teamlet."
