The Bronx, NY Veterans Affairs Medical Center is recruiting a Geriatrician to join our GeriPACT (Geriatric Patient Aligned Care Team). The GeriPACT team is a collaborative partnership with an interdisciplinary team of suitably prepared health care professionals, consisting of the GeriPACT provider, a registered nurse, a social worker, a Clinical Pharmacist, a clinical associate (LPN, LVN or health tech), dietitian, mental health providers, rehab professionals, chaplains, and clerical staff. This group is referred to as the "GeriPACT Teamlet." Other clinicians (e.g., mental health professional, dietician, occupational and physical therapist, audiologist, etc.) are also involved in clinical care as patient needs dictate.
