Apply Description The Case Management Coordinator is responsible for establishing, coordinating, and maintaining the process of inpatient admissions, pre-certifications, addressing insurance denials, collaborating with LLM and overseeing discharge planning and ensuring patients have a safe discharge plan. The Case Management Coordinator will collaborate with Intake Nurses, Utilization Review, Recreational Therapist and Social Workers, Patient/Family, Physicians, community resources and payers to ensure the patient’s progress and level of care is appropriately determined. Job Summary:      The DRN role is a non-24/7 nursing role, that crosses all continuum of health care sites functioning in the role of expert clinician, educator, and nurse care manager for diabetes patients across the continuum of care.  In collaboration with the medical and ancillary staff, monitors the clinical care of patients and provides clinical support to improve patient care and outcomes, for both inpatient, outpatient, and across the continuum of care.
