Department Description: Want to have every other week off? We are seeking a full time On Call Visit RN to work closely with the triage nurse team to support clients in the home setting from 5pm to 8am. The Community Hospice team delivers care to patients in the community, providing a wide range of skilled nursing care and support. We are an interdisciplinary team all collaborating together to support patients to remain in the setting of their choice, their home. The first 5 hours you will have intentional work such as admissions, the rest of the time you just need to be available for any urgent calls! RN II and RN IIIs are considered for this position. POSITION SUMMARY: The Office Licensed Practical Nurse II, under the general supervision of the Clinical Coordinator, the providers and the Office Supervisor, is responsible for providing optimal patient care, being accessible to patients, maintaining a high level of knowledge and working amicably with other staff members. The LPN II will maintain professionalism and strict confidentiality. BASIC KNOWLEDGE: Graduate from an approved Professional Nursing Education Program. Current LPN licensure or compact licensure recognized by the State of Vermont is required. BLS training preferred. EXPERIENCE: One to three years of hospital nursing experience or office nursing experience.
