Job Summary:     The Outpatient Behavioral Health Therapist provides quality direct and indirect patient care to individuals in the Outpatient Behavioral Health Programs.    Functions and Duties:   Maintains accurate program and client documentation, including medical records.  Conducts assessments, develops and implements a plan of treatment, in conjunction with members of multi-disciplinary team, communicates with referral sources and other appropriate parties regarding patients' progress, and coordinates patients' discharge planning; facilitating referrals and implementing smooth transitions.   Demonstrates knowledge and skills to work with diverse group of clients based upon diagnoses and age specific needs.
