Instructs, coaches, guides, and intervenes to support the patient and family in achieving the best possible outcomes while remaining in the communities they value. Promotes optimal health, well-being, and quality of life of home health patients, and their families by providing skilled, intermittent physical therapy in the patient?s home. Practices independently in the patient home using excellent, evidence-based practice for assessment, clinical decision making, clinical practice, and coordination of care across the continuum.
