POSITION SUMMARY: This position coordinates total nursing care for patients, participates in patient and family teaching, acts as a patient advocate and works as part of a team with ancillary and non-nursing departments. ESSENTIAL FUNCTIONS:  To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation.  Assesses the patient's condition and nursing care needs. Sets goals and prescribes appropriate nursing interventions to meet physical, psychological, social and rehabilitative needs of patients. Develops individual nursing care plans.
