Job Summary :   Provides patient centered navigation services and care coordination across the continuum of care. Participates in assessing, planning, implementing, and evaluating health services of a disease specific population throughout the continuum of care. Performs tasks requiring independent knowledge and judgment. Provides community outreach activities. Serves as a liaison and resource among patients, families, healthcare providers, staff, and community to provide a seamless transition throughout phases of care. Focuses on social determinants of health and relief of barriers to care. Reports data and implements quality improvement initiatives focusing on readmissions, complications, and patient self-management skills through the CaroMont Service Lines.

Training: Associate’s Degree in Applied Science from a recognized college or university.  Must have an NC license or a NC provisional license.  The provisional license will be accepted until the state boards are taken, and the results are known.  No prior work experience required.  Knowledge of occupational therapy terminology, procedures and techniques.  Considerable skill in performing occupational therapy procedures.  Thorough ability to deal tactfully with patients, physicians and hospital personnel.  Judgment is required in working with patients and performing treatments. Job Summary:  The Practice Readiness for Educational Pathways (PREP) Program provides the Respiratory Therapy student with opportunities to gain valuable experience that can help bridge the gap between theory and practice. The goal of the PREP Program is to facilitate the assimilation of the individual into the healthcare environment and culture. The PREP experience serves as a preview of direct patient care.      The student will work under the supervision of a CaroMont Health employee, licensed in the role in which the student is currently enrolled. The PREP student is assigned to a supervising employee each shift (mentor). The PREP student interns are required to schedule 12 hours of observation time per month.
