The Terraces of Phoenix, a premier 5-Star HumanGood community, is seeking an experienced Part-time Licensed Practical Nurse-Case Manager (LPN/LVN) for the Skilled Nursing Team. Reporting to the Health Services Administrator, the LPN-Case Manager is essential to ensuring a smooth, welcoming, and clinically sound transition for residents entering our community. ** 8:30 AM – 5:00 PM, Monday, Tuesday, and Wednesday ** Pay rate: $36.00 - $40.00/hour (Depending on Experience)
Essential job duties and responsibilities include, but may not be limited to:
Serve as the primary clinical liaison between hospitals, physicians, families, and internal departments.
Collaborate with therapy, pharmacy, dietary, social services, and nursing teams to support smooth transitions of care.
Participate in interdisciplinary team (IDT) meetings and assist with discharge planning as needed.
Monitor resident progress and support care coordination throughout the stay.
