Overview: The Abdominal Transplant Clinical Pharmacist provides specialized pharmaceutical care to patients undergoing kidney, pancreas and liver transplantation through all phases of transplant care, including pre-transplant evaluations, perioperative/inpatient management, discharge planning and post-transplant care. This role involves collaboration with a multidisciplinary transplant team to ensure optimal medication management, improve patient outcomes, and maintain regulatory compliance. The Abdominal Transplant Clinical Pharmacist will join an established Transplant Pharmacy team and will rotate between the kidney/pancreas and liver services, practicing in both the inpatient and outpatient settings. This role provides the opportunity to work from home one to two days per week (dependent upon eligibility, staffing levels, and clinical needs).
