Job Summary The RN - Transitional Care Coordinator is responsible for assessing patients that are medically complex and at high risk for readmission to ensure a successful transition from the hospital to community. The Transitional Care Coordinator RN is accountable to reduce the challenges that patients face post discharge and prevent hospital readmissions. Job Summary The RN - Wound & Ostomy is responsible for assessing, planning, promoting and evaluating quality patient care.   The Wound & Ostomy Nurse provides acute and rehabilitative care to people with gastrointestinal (GI), genitourinary (GU), and integumentary (skin) system wounds and disorders.  They also provide direct care to people with abdominal stomas, wounds, fistulas, drains, pressure injuries, and/or continence disorders.
