The Team Manager provides leadership guidance to the professional staff on a day-to-day basis to ensure the provision of Hospice services are consistent with the philosophy, goals, and policies of the program and MJHS. Monitors and evaluates the quality and completion of nursing documentation per the documentation policy and procedure and state and federal regulations. Facilitates and oversees the preparation and planning of the weekly IDT team meeting to ensure that all patients are reviewed per the Hospice Conditions of Participation (COP’s). Our groundbreaking hospice and palliative care programs offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care. The On Call RN Supervisor provides leadership, support, and guidance to Call Center staff for clinical/non clinical issues and situations. The Supervisor elevates complex issues to Team Manager, AOC or Hospice MD on call.  The RN Supervisor also provides quality professional telephonic intervention to patients based on the Plan of Care. Graduate from an accredited School of Nursing, BSN or BS degree preferred Three years of Hospice nursing experience Supervisory or leadership experience preferred 2-5 years Hospice experience/On Call required New York State RN licensure NYS valid license preferred Current CHPN certification preferred
