Department: Medical Clinic Days and Shifts: Full-time position Mainly Monday – Friday from 8 AM – 5 PM Description: Assist the Clinic Team (providers, nurses, support staff) by coordinating care to patients on the providers’ daily schedules and by proactively managing and coordinating care for patients not on the schedule, so as to offer quality healthcare to all clinic patients Will act as a primary conduit for the transmission of information between providers, patients, and medical record Will coordinate services for patients, including preventative care, health education, and advocacy Focuses on promotion of health, prevention of illness and injury, and protection from work-related and environmental hazards Delivers health and safety programs and services to workers, worker populations, and community groups Requirements
