JourneyCare Home Health is currently seeking a dedicated Registered Nurse (RN) Case Manager to join our home health team. This field-based position is responsible for managing a caseload of patients in Elmwood Park, IL and surrounding areas , providing skilled nursing services, coordinating care, and supporting patients on their road to recovery in the comfort of their homes. Location: Elmwood Park, IL and surrounding areas. Salary: $80,000 - $105,000 Offer based on years of experience.
