We are seeking a dedicated and compassionate Occupational Therapist to help patients with injuries, illnesses, or disabilities regain or improve their daily skills while working in a home health setting in Chippewa Falls, WI.
Minimum Requirements
Bachelor's, Master's, or Doctoral Degree in Occupational Therapy from an accredited educational program. Active Wisconsin Occupational Therapy license is required to start the assignment. At least 12 months of hospice or home health experience within the past 3 years is required. Experience with HCHB (Homecare Homebase) EMR is required. BLS certification from AHA or ARC may be required. Ability to meet productivity expectations. Client notes indicate productivity targets (example: weekly productivity points and hours expectations may apply).
Preferred.
Prior hospice-specific OT experience is preferred.
