Apply Job Type Full-time Description The purpose of the Medication Reconciliation Technician is to review, clarify, document and verify with the patient when possible, their Prior to Admission (PTA) medications. This will be done within 24 hours of admission to the hospital. Utilize the patient’s pharmacy, family, caregiver or prior office records to further investigate and verify the patient’s medication history. Confirm medications that need to be added or removed from the medication list. Once the medication list is completed, the Attending Physician will be notified that the medication list is current and verified and alert them of any changes, discrepancies or medications needing to be added or removed. The Med Rec Tech will perform these duties primarily in the Emergency Department, but also in the Med Surg units, Outpatient Surgery and the Skilled Nursing Facility as needed.
