Overview $300k+ includes Base Comp and Quality Bonus.  Plus a $50k Retention Bonus   Full-time, Monday - Friday, no weekends or evenings.  As a Physician Leader, you will have clinical oversight of 4-5 Advance Practice Providers, dividing the week 2.5 days of admin/clinical oversight + 2.5 days hands on patient care with a small panel of 140 patients.    HarmonyCares has provided home-based care for 30+ yrs.  We are a full Value Based Care model with 37 Medical Groups across 14 States & GROWING.  With multiple divisions and continuum of care services that include: Primary Care, Palliative Care, Hospice, Home Health, Diagnostics, Wound Care, Urgent Care, and Health Risk Assessments.  We manage chronic conditions of the frail Medicare population in their homes as their primary care provider and in support of other PCPs in the community.  We believe more time with the patient equals quality outcomes which has been proven through not only our growth across the U.S....
