SUMMARY OF WORK: Responsible for the overall supervision and management of care delivered to residents of the nursing home and patients of the hospital. Establishes, reviews, revises resident care plans to ensure appropriate, high quality, individualized care. Monitors resident/patient conditions, responds appropriately and in a timely manner to changes of condition.
