Overview The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to resume or adjust the activities of daily living. Collaborates with health care team and modifies plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need. This OT sees patients in Champaign/Urbana as well as areas within Piatt County. Qualifications Education Level Field of Study Bachelor's Degree Occupational Therapy Licenses/Certifications Licensed Occupational Therapist (OT) - Illinois Department of Financial and Professional Regulation (IDFPR) And Basic Life Support (BLS) within 30 days - American Heart Association (AHA) And Driver's License - Secretary of State (SOS) And Proof of Auto Insurance - Varies Specialized Knowledge and Skills Requirements Ability to work as a member of an interdisciplinary...
