What You’ll Do     The Residential Services Social Worker carries a regular caseload at the PrairieCare Residential Treatment Center, serving ages 10-18. This position focuses on facilitating groups, treatment planning, discharge planning, care coordination, care conference meetings, communication with caretakers, community providers, and others involved in the care of our patients. Collaborates daily with the multidisciplinary team to advance the plan of care.  Uphold and apply Social Work knowledge and understanding of emotional and behavioral health practices and principles. Assess, plan, implement, and evaluate the psychosocial services for patients and share social work expertise with members of the treatment team within the average length of stay of 30-90 days.
