Overview About the Role The primary purpose of the Registered Dietitian position is to assess and intervene with Adult (18-64 years) and Geriatric (65-95+ years) clients, for nutritional risk and education requirements. Responsibilities What You’ll Do Performs nutrition assessments in the time frame consistent with the N.J. State Health Code and/or policy and procedure for designated subunits, and triages clients into appropriate level of care to determine time frame for follow-up intervention and intensity of nutritional care required. Documents all nutritional care provided to clients in the medical chart using the S.O.A.P format or as outlined in the policy and procedure for designated subunits. As part of a critical access facility located within our Rural Health Clinic; the Family Medicine Physician Assistant (PA) provides acute and routine medical care, fulfilling a vital role in the community by providing high quality care to all ages. As an extension of our family practice providers, the Family Medicine PA focuses on both urgent and primary care needs; conducting patient assessments - including medical histories, physical exams, diagnostic testing, and referring to specialty services. In being part of a collaborative team, the FM PA works alongside two full-time physicians, and supports the organization by carrying out these duties in a manner consistent with the mission and vision of Coulee Medical Center.  

Family Medicine Physician with OB is a full-time position responsible for fulfilling a vital role in the community by providing comprehensive health care to all ages; emphasizing preventative health practices and disease management. C-section opportunities available! Applicants who are able to perform C-sections independently are preferred, but Coulee Medical Center is willing to train the right candidate. This position acts as the hub of the patient care team diagnosing and treating illness; providing routine examinations, referring to specialty services, and performing follow-up. In addition to providing quality patient care, the Family Medicine with OB physician provides medical direction to clinic staff as well as providing oversight and consults for Allied Health Professional Providers. As part of a critical access hospital and rural health clinic, this position operates as support for the Emergency Department and Hospital.
