Overview The Clinical Pharmacist works collaboratively with Physician teams, advanced practitioners, medical residents and staff to promote and improve safe medication use. Provides patient specific medication use teaching in high risk patients (i.e. polypharmacy, pediatric patients, patients on high risk medications or are at risk for readmission due to medication non-compliance). Assist with drug therapy management in medical emergencies (i.e. code, trauma, stroke).
