Overview
Under the supervision of a licensed pharmacist or a licensed healthcare professional (i.e. physician, nurse practitioner or nurse), the Medication Reconciliation Pharmacy Technician's primary responsibility is to perform medication history assessments for new patients coming through the Emergency Department or new inpatient direct admissions to the hospital. Medication history information will be obtained from patients directly or the patient's family member/caregiver, retail pharmacy or physician office. The Medication Reconciliation PharmacyTechnician will enter medication histories into the electronic database or will update the database if the patient has existing medication information--this involves compiling and verifying patient medication lists, comparing those lists with current prescriptions, and communicating any discrepancies or questions to the patient's nurse, nurse practitioner and/or other healthcare providers.
