It helps to identify and address any medication access barriers to ensure that patients have their medications and are counseled on appropriate use prior to their discharge. The primary goals of this service are to help facilitate a smooth transition out of the hospital, promote discharge therapy adherence and help prevent non-adherence related readmissions. A Transitions of Care Pharmacist will be responsible for the quality assurance of the pharmacy services to patients discharging from the Medical Center.
