JourneyCare Home Health is currently seeking a dedicated Registered Nurse (RN) Case Manager to join our home health team on a PRN basis. This field-based position is responsible for managing a caseload of patients in Elmwood Park, IL and surrounding areas , providing skilled nursing services, coordinating care, and supporting patients on their road to recovery in the comfort of their homes. Location: Elmwood Park, IL and surrounding areas. Hourly: $38.46 - $50.48 Offer based on years of experience.
