Overview: The Diabetes RN/RD, CDCES in conjunction with multidisciplinary team members of the UF Health Pediatric Endocrinology outpatient diabetes clinic, is responsible for providing ongoing compassionate care for patients and families affected by diabetes.   The Diabetes RN/RD:   Conducts assessment of each patient's/family's baseline knowledge of and adherence to their diabetes management regimen to determine their educational needs required to manage diabetes, and provides appropriate ongoing education in individual, group, and clinic settings.   Provides Diabetes Self-Management Education and Support (DSMES) within the scope of practice of the RN/RD as outlined by the American Diabetes Association Recognized Education Program Standards and according to department curriculum.   Creates and maintains diabetes education materials used for individual and group education.   Acts as a liaison between the patient/family and MD/APRN by providing ongoing patient support via...
