Description Summary: Breast Care Patient Navigator Nurse serves as a main point of contact for patients, caregivers, and physicians to provide resources and assistance with accessing clinical and supportive care services offered within the healthcare system and in the community. The Nurse Navigator is an advocate for the patient in their dealings with the healthcare community, helping patients navigate their way to timely and quality care, assisting the patient throughout their diagnosis/treatment, and aiding in overcoming any barriers to care. Facilitates appointments for consults and support services within established standards. Functions as a member of an interdisciplinary team to assess, secure, and coordinate appropriate services.
