If you are passionate about making a difference in patient care and have experience in Transition Care or Discharge Planning, along with a background in an acute care hospital setting, we would love to hear from you. As part of our Weekender Incentive Program (WIP), you are required to work 3 out of 4 weekends in a four-week schedule.
