Our highly trained team provides dependable services and support when it is needed most. Our goal is to provide care for children with chronic, complex conditions in their home and allow them to stay in school—and out of the hospital—where they can thrive. Provides, coordinates, teaches, and/or supervises care for a group of patients usually in the setting of the patient’s home. Employs the appropriate discipline specific process to develop and implement plans of care for patients requiring in‐home health and support services, under physician orders.
