ChristianaCare Hospital in Newark, DE, is seeking an RN Case Manager with experience in Discharge Planning in an Acute Care Hospital Setting. The RN Case Manager will coordinate care and drive patient progression to establish a discharge plan. This includes functioning as an interdisciplinary team member, creating, implementing, and developing all aspects of discharge planning to establish appropriate, timely, and effective transitions throughout the care continuum.
