Overview Provides advanced nursing clinical care to patients in the VNS Hospice Care program consistent with VNS Health philosophy and standards of nursing practice in relation to VNS Health goals and objectives and improves hospice utilization for hospice eligible patients. Works under general supervision. • Provides advanced nursing care and treatment to hospice patients in both the home and via telephonic visits, which includes assessment, diagnosis, prescribing medication, ordering and interpreting appropriate tests, and ordering treatments and DME. • Performs in-person pre-admission F2F visits when necessary, for certification of terminal illness by the Hospice Team Physicians and complete hospice admission to the Hospice Program when deemed hospice eligible. Provides medication management and assessment on an ongoing basis. Provides consultation and education to other agencies and disciplines, including other VNS Health services. Assists in the overall management of the Behavioral Health program. Acts as a key resource in providing clinical and operational guidance and support to assigned teams and other staff to achieve and enhance team outcomes. Provides advanced nursing clinical care for clients in accordance with current State and Federal rules and regulations for nurse practitioner scope of practice and national standards of care, and specific program regulations/contract requirements.
