Telemedicine Provider (NP/PA) - Remote Theoria Medical is a physician-led organization redefining post-acute care across the country. Role Summary The Telemedicine Provider delivers high-quality virtual medical care to residents in skilled nursing facilities, long-term care facilities, assisted living communities, and post-acute care settings. As a dedicated remote advanced practice provider, you'll manage acute and chronic medical concerns, support admissions and discharges, facilitate care transitions, and serve as a trusted clinical resource for facility partners, all through our telemedicine platform.
