The Nurs Practitioner(NP)/ Physician Assistant (PA) is part of Dana-Farber Cancer Institute’s Cancer Care Access Program (CCAP), which aims to place Dana-Farber at the forefront of efforts to reduce differences in cancer outcomes across local communities. The NP/PA will provide care to patients in CCAP’s onsite Cancer Diagnostic Clinics located within Federally Qualified Health Centers (FQHCs) throughout the Greater Boston area. The NP/PA will evaluate adult patients referred by primary care providers at participating FQHCs for new, existing, or prior cancer diagnoses; benign hematologic conditions; breast concerns; and other concerning cancer-related symptoms. Clinical responsibilities include cancer screening, diagnostic workups, follow-up care, reconnection to cancer care, referrals for second opinions, cancer survivorship care, and patient education.
