Patient Care: Before the first treatment on any field (initial or boost) the therapist will do the following: check the calculations sheet and simulation sheet with the record and verify system to assure that all of the entries have been correct They will initial the calculation sheet and place an entry in the treatment chart stating that they verified the initial IMPAC set-up and simulation information. The chart will be checked to make sure that there is a signed consent form, face photo and simulation pictures are present. They will review the simulation film before the first treatment. They will record the initial set-up date on treatment parameters sheets and report any deviation to physicist/dosimetry. Initial port film is to be taken on the first treatment if the treatment blocks are available.
