Position Summary : The Caregiver Coach provides guidance, education, and support to primary caregivers of clients enrolled in the SFC/ATTC Medicaid Waiver Programs. This role is responsible for assessing the needs of both the participant and caregiver, developing individualized coaching plans, and delivering ongoing training and support. The Caregiver Coach monitors progress, conducts regular reassessments, and adjusts coaching strategies as needed. This position also develops and delivers relevant training materials, resources, and tools to support caregivers in effectively managing participant care and maintaining their own well-being. At a minimum, the Caregiver Coach engages with caregivers on a bi-quarterly basis to assess evolving needs and provide strategies related to participant care, behavior management, and caregiver self-care.
